I/we (dancer and parent/guardian) understand that participation in dance classes and activities could result in some possible personal injury. Despite precautions, however accidents and injuries may occur. By signing this release form, I/we assume all risks related to the use of any and all spaces used by Sarah Daurie Dance Company (SDDC).

I/we agree to release from responsibility Sarah Daurie Dance Company, including all teachers, dancers, staff members and facilities used by both entities from any cause of action, claims, or demands now and in the future. I/we will not hold Sarah Daurie Dance Company liable for any personal injury including scrapes, bruises, cuts, sprains, fractures, broken bones, concussions or death or any personal property damage/loss, which may occur on the premises before, during or after classes.

Furthermore, I/we agree to respect the class and facility rules and take full responsibility for my/our behaviour in addition to any damage I/we may cause to the facility used by Sarah Daurie Dance Company.

I understand that Sarah Daurie Dance Company is an insured, licensed and accredited organization. In the event that I/we should observe any unsafe conduct or conditions before, during or after classes, I/we agree to report the unsafe conduct or conditions to Sarah Klymchuk (owner), instructors or staff members as soon as possible.

PHOTOGRAPHY/VIDEO – I give permission to have my child’s photographs/videos to be used for publicity, advertising and acknowledgments in the studio, on the SDDC website and SDDC social media.

RULES & REGULATIONS

  • Students arrive on time and prepared for class.
  • Street shoes must be removed before entering the studio.
  • Only water is permitted in the dance spaces.
  • No chewing gum.
  • Respect the dance space by cleaning up after yourself, not touching lights, equipment or thermostats.
  • No students or parents are permitted behind the desk or in the storage rooms.
  • Be respectful of others by using polite behaviour and proper manners.
  • No cell phones in class.
  • No recording or photography without permission.
  • Students must be properly dressed for class following our dress code.
  • Disrespectful behaviour towards teachers, parents and fellow students will not be tolerated.

PAYMENT & WITHDRAWAL POLICIES

  • SDDC accepts cash, e-transfer, debit and all credit cards
  • Accounts must be paid in full prior to next season’s registration
  • For families paying in full prior to August 1, 2025 there is a 5% discount.
  • Families who pay in 10 installments there will be a $10 administration fee added to your fees.
  • A late fee of $15 will be applied for every week that an invoice remains unpaid.
  • There is a $25 plus GST registration fee per dancer due at time of enrollment, this fee in non refundable.
  • Refunds will not be issued after September 30, for dancers who withdraw from classes a written notice must be submitted two weeks prior to withdrawal date. There is a $26.25 withdrawal fee.
  • Competitive dancers will also have additional fees associated with their performances. These include specialty choreography (solo, duo/trio), costume and entry fees.
  • Non competitive students will have costume fees, as they perform in our year end recital.
  • THERE ARE NO REFUNDS FOR ENTRY, CHOREOGRAPHY COSTUME OR RECITAL FEES.
